Fermanagh Active Living Project
Description
This is a secondary prevention project, it's aim is to encourage medically vulnerable clients to take up tailor-made exercise in order to improve their health and wellbeing, ie medically vulnerable clients which are statistically more prone to coronary heart disease ie clients with diabetes, controlled hypertension and/or obesity are referred for an individually tailored/supervised exercise.

Objectives

To encourage GP/Practice nurses to refer medically suitable clients into the scheme

Clients will see the value of/enjoy being physically active.
There will be medical benefits is reduction in HBA1c, lower blood pressure, weight loss, reduction in medication.
No clients will be discriminated in terms of age, physical ability, race or ethnicity.
Actual
Over 200 clients were referred in the first nine months of the project.
Clients are staying with the project (the client received six months free exercise and six month's half price exercise) and those finished the project continue to exercise.
The majority of clients exercise twice a week with the project and twice a week on their own.
A satisfaction survey has shown the clients enjoyed the project.
Whilst it is too early to state that there have been definite medical changes a number of clients have lost a significant amount of weight, HBA1c results appear more stable, one client has reduced his oral diabetic medication from two tablets daily to 1/2 a tabley daily.
The age range of clients on the prject (it is open to adults only) is from 16-72 years.
Clients involved in the project have been empowered to talk to others about the project ie update the local GPs formally and talk to Prince Charles.
Link with other projects ie Fit Foods, New Horizons, SMI research, the pre-assessment clinic.
Medical improvement and fitness improvement noted.
