Neurovascular risk factor management through a nurse-led clinic

Description

This nurse led clinic targets all people who have had a transient ischaemic attack (TIA, mini stroke) or who have been assessed at the medical neurovascular clinic. This nurse-led clinic is open for any discipline to refer.
People who have had a TIA can also self refer. At this clinic I advise individuals and their families on the modifiable risk factors for stroke and together we put in place strategies to deal with each individual risk factor and work through any lifestyle issues. The clinic runs once to twice weekly depending on demand. If required I have immediate access to medical staff. I also refer to other disciplines within the hospital and community setting and liase with GPs. I review individuals until I feel their risk factors are controlled and the person is confident to continue without support. Each individual who attends this clinic is given the contact number of the Nurse Specialist for Stroke for the telephone advice line.

Objectives

1. To identify individuals at high risk of stroke.
2. To provide a nurse led service which focuses on early secondary prevention through modification of Risk Factors and Lifestyle issues.

3. To provide a comprehensive educational programme on the management of risk factors for all individuals who have suffered TIA/Stroke.

4. To enable individuals to manage their risk factors through empowerment, education and support and make lifestyle choices.

5. To ensure that individuals are satisfied with the nurse led service provided.

6. To provide each individual with a contact number for the neurovascular-led telephone advice line.

Actual

This clinic to date has seen 139 people post transient ischaemic attack and stroke for risk factor management and lifestyle modification. To date, I have carried out over a six month period a client satisfaction survey to evaluate the effectiveness of this clinic and the benefit and support to the individuals who have attended this clinic.
