Description
Standardised questionnaire completed on an individual basis, to assess (and ultimately improve) the quality of life (QOL)of service users. Would be received regularly - based on QOL-QR Schalock and KO Keith.

Objectives
1. Improve the overall quality of life, ensure service users received/had access to ordinary services and services they were entitled to.
2. To improve and maintain healthy well being to offer informed choice in all aspects of daily living.
3. Highlight areas of customer dissatisafaction.
4. Identify opportunities for improvement.
5. Ensure that service users receive the best service.
6. Results shared at tenants meeting to allow for peer support.
7. Share good practice with others within the service.
8. Allows the department to assess if 'we are doing what we say we are doing.

To have individual results that would be incorporated into BILD and person centred plan to allow for areas of improvement to be hihglighted and acted on.
To see if this pilot study could be adapted to be used in other areas of the Trust.
To allow service users to air grievances in contrived, supported way.
